WHAT IS YOUR NEW PATIENT PROCEDURE?

Getting help is a lot easier than most people think! Almost anyone can receive services from LaPaz! In order to start treatment, a patient must complete an intake packet and initial assessment. Both can be set up by the office staff through a phone call or office visit! Our website allows for booking requests as well. If a patient prefers to book with the website, office staff will be in contact within 2 business days to confirm an appointment.

DO I QUALIFY TO RECEIVE SERVICES?

Although our services are catered to at-risk youth, we are happy to assist anyone in the community. Walk-In patients are accepted and welcomed!

WHAT IF I DON'T HAVE INSURANCE?

Although insurance is our main form of payment, patient can set up a different payment if needed. For more information, contact us at (318)224-9200

WHAT IS AN INTAKE PACKET AND INITIAL ASSESSMENT

An intake packet is a collection of forms used by our Admin staff to collect and track patient private information required by the state.
The Initial Assessment is a 30-45 minutes one-on-one sessions with a Licensed professional. It is used to develop a unique treatment plan for every patient.
